Symptom narrative

The office realized they gave an expired Arexvy after administration; This non-serious case was reported by a other health professional via sales rep and described the occurrence of expired vaccine used in a 63-year-old patient who received RSVPreF3 adjuvanted (Arexvy) (batch number 3M9DS, expiry date 18-NOV-2025) and (batch number ZJ9FF, expiry date 26-NOV-2026) for prophylaxis. On 02-DEC-2025, the patient received the 1st dose of Arexvy (unknown deltoid). On 02-DEC-2025, an unknown time after receiving Arexvy, the patient experienced expired vaccine used (Verbatim: The office realized they gave an expired Arexvy after administration). The outcome of the expired vaccine used was not applicable. Additional Information: GSK receipt date: 05-DEC-2025 The office had administered the first dose of Arexvy before realizing that the vaccine was expired. They discovered the issue only after the administration was complete. Prior to giving the dose, staff had checked the expiry date on the accompanying adjuvant, which was valid until 26th November 2026 and assumed the entire product was in date. It was only afterward that they noticed the lyophilized antigen component had already expired.
