A 37-year-old female patient received pneumococcal 20-val conj vac (dipht CRM197 protein) (PREVNAR 20), on 01Dec2025 as dose number unknown, 0.5ml single (Lot number: MA2505, Expiration Date: 30Sep2026) intramuscular, in left deltoid for immunisation. The patient's relevant medical history included: "Hypogammaglobulinemia" (ongoing); "chronic sinus infections" (ongoing); "Asthma" (ongoing); "Stage 4 deep infiltrating endometriosis" (ongoing); "migraine with aura" (ongoing); "Enchondroma", start date: Dec2013 (ongoing), notes: Enchondroma is in my right fourth and fifth ribs. Chronic joint and bone from previous endometriosis. Concomitant medication(s) included: AIRSUPRA [BUDESONIDE;SALBUTAMOL]; TIRZEPATIDE; FUROSEMIDE; PHENERGAN [PROMETHAZINE HYDROCHLORIDE]; UBRELVY; NURTEC taken for migraine; ZOFRAN [ONDANSETRON] sublingual; METHOCARBAMOL taken for endometriosis; GABAPENTIN; HEATHER [CALLUNA VULGARIS] taken for endometriosis. Additional information: The patient called regarding Prevnar 20, which she had been administered on 1 December 2025 and experienced side effects shortly afterward. The side effects included headache, muscle ache, fever up to 102.7°F, lethargy, severe arm pain, injection site swelling, redness, raised, hot, itchy arm, and shortness of breath; the fever had resolved at the time of reporting. The patient reported having hypogammaglobulinemia, chronic sinus infections, and asthma, and stated that immunology recommended the vaccine. Her medical history included Stage 4 deep infiltrating endometriosis, migraine with aura, asthma, and enchondroma of the right fourth and fifth ribs. She reported chronic joint and bone pain from previous endometriosis treatment, for which she was receiving tirzepatide. Within two weeks prior, she had undergone blood work for immunology, including pneumonia antibody testing, which was abnormal, showing no immunity, and an allergy profile, which revealed eight separate allergic responses. All adverse events started on 2 December 2025 and continued through the time of reporting. She had taken ibuprofen to relieve aches and fever while waiting for medical guidance.
