The patient's medical history, concurrent conditions and concomitant therapies were not reported. On 29-Aug-2025, the patient was vaccinated with Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD), (lot #Z002989, expiration date: 14-Jul-2026) 1 dosage form. total (route, dose, and dose number were unspecified) for Prophylaxis (Product storage error). The vaccine was reconstituted with sterile diluent (Hollister-Stier STERILE DILUENT), 1 dosage form, total, (lot # and expiration date were not reported) (lot#, expiration date, route, dose, and dose number were unspecified) for prophylaxis, administered on the same date. The vaccine was administered after being involved in a temperature excursion. The administered dose of the vaccine experienced a temperature excursion 21.9-degree Fahrenheit; 14-degree Fahrenheit to 46-degree Fahrenheit for 12 hours, 30 minutes; 6-degree Fahrenheit to 14-degree Fahrenheit for 37 hours, 30 minutes with no previous temperature excursion reported.
