Wrong vaccine administered; four-year-old male a T-DAP (Boostrix); This non-serious case was reported by a non-health professional via call center representative and described the occurrence of wrong vaccine administered in a 4-year-old male patient who received DTPa (Reduced antigen) (Boostrix) (batch number F9K3L, expiry date 20-NOV-2027) for prophylaxis. Co-suspect products included DTPa (DTaP vaccine) for prophylaxis. On 08-DEC-2025, the patient received Boostrix (right thigh). On an unknown date, the patient received DTaP vaccine. On 08-DEC-2025, an unknown time after receiving Boostrix and not applicable after receiving DTaP vaccine, the patient experienced wrong vaccine administered (Verbatim: Wrong vaccine administered) and inappropriate age at vaccine administration (Verbatim: four-year-old male a T-DAP (Boostrix)). The outcome of the wrong vaccine administered and inappropriate age at vaccine administration were not applicable. Additional Information: GSK Receipt Date: 08-DEC-2025 The reporter reported that a four-year-old male was administered with a T-DAP (Boostrix) instead of a D-TAP in his right upper thigh. The batch number was not provided and a request for this information has been made.
