Symptom narrative

Patient walked in for a flu shot. The pharmacist administered the Flu shot, however, when rph was entering it into the system , the flu shot was already given to the patient in October by another provider. When we called the patient, he does not recall receiving the vaccine and this is why he came to receive this vaccine.
