The team member received a verbal order from the physician to administer the influenza vaccine. The order was verified through a verbal read-back to the physician prior to administration. The vaccine was administered before scanning for documentation. Following administration, the team member was unable to release the order or scan the vaccine into the system. The incident was promptly reported to the RN. Upon chart review, it was determined that the patient had previously received an influenza vaccine in September.
