Symptom narrative

A 6month old patient received a second dose of RSV vaccine. I consulted newborn discharge summary and noted MOC had not received the RSV vaccine during pregnancy but both myself and my MA misread CIIS/the immunization module and did not correctly identify that this baby had already received a dose of RSV vaccine in October.
