Site: Pain at Injection Site-Mild, Systemic: numb-Medium, Systemic: Numbness (specify: facial area, extremities)-Medium, Additional Details: pt said felt numb at injection site, confirmed it is NOT the whole arm. Pt had shingrix shot before, this is second dose. no record on Hep B vaccine, both given same arm (right). pt confirmed no other pain or itch, no swelling of body and no issue with breathing. Counseled pt to seek medical help if numbness not going away (phone call was at 420pm.
