Symptom narrative

Systemic: Confusion-Mild, Systemic: Dizziness / Lightheadness-Mild, Systemic: Fainting / Unresponsive-Mild, Additional Details: Patient received both flucelvax and covid vaccines in the same arm as he has done in past flu seasons. After the immunizer administered the second vaccine and was placing the needle in the container the patient fell forward coming out of the chair and hitting his nose and forehead on the floor. He briefly lost consciousness. An ambulance responded and brought hin to the ER to get checked out. All vitals were normal.
