Symptom narrative

Site: Itching at Injection Site-Medium, Site: Pain at Injection Site-Severe, Site: Swelling at Injection Site-Severe, Site: Shoulder Joint Injury (prolonged pain, tingling, etc.)-Severe, Additional Details: Patient received vaccine 11/25/25 and noticed she got the injection too close too right shoulder. felt pain immediately but got severe on friday 11/28 when she had to go to ER bc she lost complete range of motion (unable to do daily activities). Prescribed steroids by ER and feeling a bit better now but not complete range of motion.
