Patient Did Not Receive Second Dose; Initial report received on 26-Nov-2025. A pharmacist reported that a patient received the first dose of HEPLISAV-B (hepatitis B vaccine) for hepatitis B immunization in 2022 and did not receive the second dose. No medical history or concomitant medications were reported. On an unknown date in 2022, the patient received dose 1 of a hepatitis B vaccine [HEPLISAV-B (lot number, expiration date, NDC number, route, and site not available)]. At the time of reporting, the patient did not receive the second dose yet. No other information was available.
