administration of Kinrix in a 2-month-old instead of Pediarix; This non-serious case was reported by a physician via call center representative and described the occurrence of wrong vaccine administered in a 2-month-old female patient who received DTPa-IPV (Kinrix) (batch number y49bz, expiry date 17-MAY-2026) for prophylaxis. Co-suspect products included DTPa-HBV-IPV (Pediarix) for prophylaxis. On an unknown date, the patient received Kinrix and did not receive Pediarix. On an unknown date, an unknown time after receiving Kinrix, the patient experienced wrong vaccine administered (Verbatim: administration of Kinrix in a 2-month-old instead of Pediarix). The outcome of the wrong vaccine administered was not applicable. This report is made by GSK without prejudice and does not imply any admission or liability for the incident or its consequences. Additional Information: GSK receipt date: 12-DEC-2025 A physician called in to request data about administration of Kinrix, as the patient's parents did not want Hepatitis B component. No adverse events have been reported. The vaccine administration facility was the same as primary reporter.
