Symptom narrative

pt is 74y/o. Pt arrived to pharmacy to get a scheduled vaccine. Upon scheduling, patient selected to receive fluzone HD. Pt said at counter that she informed associate that she cannot get HD and wants regular dose because she has has a autoimmune dx. Flublok was dropped instead and was administered on 11/4. Pt came in today on 12/17 explaining that we gave her the wrong shot. She said that she cannot have HD and that she wanted a normal dose. I told her that we did not give her HD and that flublok is one of the flu shots given to patients under 65 y/o and is considered as normal dose. she told me that she did research and it shows that flublok has higher concentration compared to fluzone and because of that it caused her health problems such as UTI (happened 1 week post shot), hives (happened a few weeks post shot), and sensations in the mouth making her believe that she needed a root canal (happened within 6 days after shot). Pt also received flublok the year prior 2024-2025 and did not have any health problems.
