Patient received an extra Hepatitis A vaccine. Parents have shared custody and patient splits time between [state withheld] and [state withheld]. When patient came for a new establish visit on 11/3/2025 mom brought what she thought was the most up to date record from her pediatrician in [state withheld]. Vaccines were entered into system accordingly patient was given Hepatitis A. Patient received Hepatitis A doses on 1/5/2024 and 7/18/2024 at Pediatricians in [state withheld]. The immunization record given to staff did not reflect the dose given on 1/15/2024. Patient had no adverse reactions.
