Symptom narrative

Patient stated that he has received Spikevax (Moderna) on 03/18/2021, 04/05/2021, 09/12/2022, 10/17/2023, 12/10/2024 and 06/11/2025. He had a wound/bone spur many years before the vaccine that would not heal and had several abscess after the covid vaccines. 1st abcess in foot required to go to infusion center for 6 days a week for antibiotics and had a surgery on May 1st of 2023. 2nd abcess in scrotum surgically removed on 04/02/2023. 3rd abcess in per-anal region on 06-27-2025, drainage tube inserted. Pilonidal Cyst was surgically removed on in Sep 2025. 3rd surgery on 10-28-2025 seton removal/advanced flap surgery.
