Symptom narrative

Patient reported persistent left deltoid pain after injection and has persisted for the past 6 weeks. Was seen 18 Dec for ongoing pain and preliminary diagnosis of injury of the biceps tendon (long head). Was referred to physical therapy for further evaluation and treatment.
