Caller stated no patient symptoms have been reported; expired dose of VAQTA was administered to a patient. Caller stated the dose expired 11/26/2025 and was administered 12/17/2025.; This spontaneous report was received from a nurse and refers to a patient of unknown age and gender. The patient's medical history was not reported. The patient's concurrent conditions were not reported. Concomitant therapies were not reported. On 17-Dec-2025, the patient was vaccinated with an expired dose of Hepatitis A Vaccine, Inactivated (VAQTA) (dose, route of administration, anatomical location was not reported, lot number Y008338, which was verified to be valid, expiration date established on 26-Nov-2025) for prophylaxis (Expired product administered). No patient symptoms have been reported (No adverse event).
