Inappropriate Reconstitution Technique; This non-serious case was reported by a pharmacist via call center representative and described the occurrence of inappropriate reconstitution technique in an unspecified number of patients who received Herpes zoster (Shingrix) (batch number 3272H, expiry date 31-MAR-2027) for prophylaxis. On 15-DEC-2025, the patients received Shingrix. On 15-DEC-2025, an unknown time after receiving Shingrix, the patients experienced inappropriate reconstitution technique (Verbatim: Inappropriate Reconstitution Technique). The outcome of the inappropriate reconstitution technique was not applicable. Additional Information: GSK Receipt Date: 15-DEC-2025 On 15-DEC-2025, a reporter reported that she had called to get another diluent for a Shingrix dose since the diluent was wasted accidentally. She confirmed that no patient had double diluent when the vaccine was administered, and she did not want to waste the lyophilized part.
