Vaccine mix up/the Pediarix was mixed with the antigen for the hib Hiberix; This non-serious case was reported by a nurse via call center representative and described the occurrence of inappropriate preparation of medication in a 6-month-old female patient who received Hib (Hiberix) (batch number Y3H23, expiry date 25-AUG-2026) for prophylaxis. Co-suspect products included DTPa-HBV-IPV (Pediarix) (batch number 2TP34, expiry date 19-SEP-2027) for prophylaxis. On 15-DEC-2025, the patient received the 3rd dose of Hiberix and the 3rd dose of Pediarix. On an unknown date, an unknown time after receiving Hiberix and Pediarix, the patient experienced inappropriate preparation of medication (Verbatim: Vaccine mix up/the Pediarix was mixed with the antigen for the hib Hiberix). The outcome of the inappropriate preparation of medication was not applicable. Additional Information: GSK Receipt Date: 15-DEC-2025 Clinical Nurse supervisor reported that a 6 month that they gave, the Pediarix and Hiberix, the way that our Hiberix comes was supplied as the antigen vial and the reconstitution solution the saline, so the Pediarix was mixed with the antigen for the hib and obviously they gave the hib as the sterile saline so no medication in that one so the pediarix and hiberix was mixed together in one vaccine. With the mix of these they asked to need to revaccinate for either of them.
