Only diluent administered; Only diluent administered; This non-serious case was reported by a physician via call center representative and described the occurrence of inappropriate preparation of medication in a 16-year-old female patient who received Men ABCWY NVS (Penmenvy) (batch number 5S723-B, expiry date 30-MAR-2028) for prophylaxis. On an unknown date, the patient received the 1st dose of Penmenvy. On an unknown date, an unknown time after receiving Penmenvy, the patient experienced inappropriate preparation of medication (Verbatim: Only diluent administered) and inappropriate dose of vaccine administered (Verbatim: Only diluent administered). The outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were not applicable. Additional Information: GSK Receipt Date: 15-DEC-2025 The reporter reported about Penvemy that we have the VFS, the nurse forgot to put the powder (ACWY) into the Men B liquid (ie, Men B of the Penvemenvy part confirmed and not Bexsero given) and gave it to the patient, she was 16 and this was her first dose. The reporter asked can we do penmenvy for the 2nd dose, would it count or will bexsero count, do we need to redo can we do penmenvy in 6 months would that be ok.Because usually the recommendation was Penmenvy 1st and Bexsero 2nd in 6 months or so or Penmenvy 6 months apart. The reporter did not see anything that says the opposite way, that you can give bexsero and then Penmenvy although it's the same. The batch number was not provided upon follow- up with the reporter.
