Symptom narrative

Approximately 40 minutes after vaccination patient broke out in an itchy rash on upper chest neck and cheeks. Patient was given Benadryl 25 mg PO. 30 minutes after dose itching had resolved but chest and face were still red. Patient was given Claritin 10 mg PO. One hour after Claritin dose, rash and redness was gone.
