Pneumococcal Conjugate PCV13 (Prevnar 13) was administered at family medicine on 03/07/2022 and 06/06/2022. Hib-acthic/omnihib/hiberix (PRP-T) was administered at family medicine on 03/07/2022 and 06/06/2022. Hep A (Ped/adol) was administered at family medicine on 06/06/2022. DTap-Hep B-IPV (Pediarix) was administered at family medicine on 03/07/2022. I as patient's mother had expressed patient's regression after the vaccinations on 03/07/2022 and expressed concern that he is having a reaction to the vaccinations. Employee verbally scolded me and told me that by not vaccinating patient on 06/06/2022, I would cause more harm to him. I tried to ask questions about what is in the vaccines and what reactions the vaccinations can cause and employee told me the vaccines are safe and effective and do not cause the symptoms patient was showing after the vaccinations he had on 03/07/2022. After patient's vaccinations on 03/07/2022 we seen by 03/15/2022 that patient no longer was making eye contact for connection and when calling his name. By 03/26/2022 he stopped completely. After patient's vaccinations on 06/06/2022, patient's symptoms did not improve. On 06/14/2022 patient had a rash of bumps that started on his neck and traveled to his whole body. By 06/21/2022 patient still had recurrent otis media, this time of his right ear. On 09/13/2022 patient has body shaking/tremors. On 07/13/2023, patient had tubes placed in both ears. The providers verified that the tubes improved his hearing to normal. After 07/13/2023, patient's hearing improved but his emotional, mental, physical cognitive connections did not change. Patient was still unresponsive to his name and connection in any way/shape/form. At this point and previous to the tubes placed on 07/13/2023, patient progressed into a complete unresponsive state. Completely unaware of his surroundings as well as himself, he could verbally and physically stim, can no longer speak words and could not follow any direction or hold any connection with his eyes or with physical connection in any way. We were not able to hug him without him getting irritable, we could not read to him or play with toys without him getting irritable. Signing would cause him to be irritable. Patient would stim with verbal tones and his hands and head/neck.
Patient was ill on 02/16/2022-he was diagnosed with upper respiratory illness and acute left otitis media and was prescribed amoxicillin (Amoxil) to take on 02/16/2022 moving forward to resolve the problem.
Patient was born with a high impeforated anus and since has been given a manmade anus that will never be normal but allows stool to pass through. He was also born with a heart murmur which has since been notated as a small residual left to right atrial shunt due to a small atrial septal defect and will not need any intervention and should resolve as he grows.

On 11/24/2023 patient tested positive for Autism level 3, regressive/non verbal from Provider at behavioral health. On 07/24/2023 patient tested negative for genetic Autism with provider -showing he is not genetically Autistic. On 10/02/2025 patient tested negative again for genetic Autism with provider showing he is again, not genetically autistic.
