Symptom narrative

Pt may have been given the saline mixture of Menveo without the powder being mixed in. No way of knowing since more than one pt got this vaccine during this month and discrepancy was noted during count. Health Department has been notified, and patient will be offered additional vaccine dose but per Health Department is not needed.
