HEPLISAV-B Given to 16 Year Old Female; Initial report received on 01-Dec-2025. A nurse reported that a 16-year-old female received HEPLISAV-B (hepatitis B vaccine) for hepatitis B immunization. No medical history or concomitant medications were reported. On 01-Dec-2025, the patient received dose 1 of a hepatitis B vaccine [HEPLISAV-B (lot number, expiration date, NDC number, route, and site not available)]. The patient was 16 years old at the time of administration. No other information was available.
