THERE SO FAR HAS BEEN NO ADVERSE REACTION TO OUR KNOLEDGE, it was brought to our attention that the patient received a HD flu vaccine yesterday at the community health clinic. The patient has limited language speaking ability and came to the counter requesting a Flu Vaccine. The state immunization that we checked did not show him obtaining one this year, however, he had two files in the state immunization system. I spoke to the clinic nurse, he was sent in to obtain a COVID and Pneumonia Vaccine per their records, unfortunately this is based on lack of clear communication
