Symptom narrative

My brother received the covid vacination on Sunday, December 7, 2025 at 10 am. I just know it was Moderna unknow which one. I talked to patient on the phone on Wedesday, December 10, 2025 and he stated he wasn't feeling well. I visited him on Friday, December 12, 2025 at skilled nursing facilityand he stated he was not feeling well, he had an upset stomach. On Saturday, December 13, 2025 patient was transported Hospital emercency room at around 3 pm because he had fallen and had shortness of breath. I got a call from a nurse around 6:40 PM to tell me they were trying to resuscitate him. The nurse called a little later to inform me that patient had passed away.
