off label vaccine - prescribed for inappropriate age; This non-serious case was reported by a consumer via call center representative and described the occurrence of intercepted product prescribing error in a 46-year-old female patient who received Herpes zoster (Shingrix) for prophylaxis. The patient's past medical history included breast cancer (Breast cancer - treated five years ago) and shingles (Shingles). Additional patient notes included the patient was not immunocompromised. The patient did not receive Shingrix. The patient experienced intercepted product prescribing error (Verbatim: off label vaccine - prescribed for inappropriate age). The outcome of the intercepted product prescribing error was not applicable. Additional Information: GSK Receipt Date: 16-DEC-2025 The patient's doctor had written a prescription for Shingrix vaccination (adverse event), but her pharmacy declined to administer the vaccine. The batch number was not provided, and unable to contact the reporter.
