Site: Bruising at Injection Site-Mild, Additional Details: Patient called on 12/12/25 to get info on flu shot received on 12/11/25 but only covid shot was shown administered on her profile. Explained that she already received Fluzone HD on 08/25/25.Verified w/staff that she did administer both covid & fluad to the patient on 12/11/25 but flu shot is not showing it was entered. Patient received both fluzone hd (08/25/25) &fluad (12/11/25) per immunazation history, patient and Staff. No injury/bruising was stated(form would not let me submit w/out)
