Symptom narrative

Site: Pain at Injection Site-Mild, Systemic: Dizziness / Lightheadness-Mild, Systemic: Fainting / Unresponsive-Mild, Systemic: Vomiting-Medium, Additional Details: Patient passed out for a few seconds after receiving second shot, and then woke up. He asked for a bag and started vomitting. He told me that this happened before and I asked if he needed an ambulance. He initially said no, and stayed sitting in the pharmacy and vomitted a few more times. I asked if there was someone I could call to pick him up to which he said no. Later on he asked for an ambulance and I called 911 right away.
