Systemic: Dizziness / Lightheadness-Mild, Systemic: Fainting / Unresponsive-Medium, Systemic: Headache-Mild, Systemic: Nausea-Medium, Systemic: Shakiness-Medium, Systemic: Vomiting-Medium, Additional Details: Before vaccine admin pt states that she has had dizziness/fainting with a previous vaccine (dad unaware). 2-3 minutes after vaccine is delieverd, pt reports dizziness, get her a chair. Sits and faints shortly after. Layed pt slowly to the floor, woke up approximately 30 seconds later, has what I assume is a panic attack. Dad declines emergency services. Pt reports dizziness/nausea. 30mins later throws up. F/u with pt this am and did not sleep well, consumed soup, is doing better this am
