Symptom narrative

MMRV(Proquad) administered instead of MMRII, at the same time as Varivax, causing two Varicella doses to be given at once. Guardians made aware on 12/16/2025 when error caught. Parents stated patient had a fever for four days but had resolved prior to phone call. No other abnormalities/abnormal findings noted per parents.
