Symptom narrative

Patient was into clinic to receive second HPV and Hepatitis A vaccines. Patient is a 13-year-old female with no insurance. Vaccines were given. Patient was educated on potential side effects of the HPV vaccine and advised to remain in clinic for 15 minutes post vaccination for monitoring. After all vaccines had been given, it was discovered that the patient was give a HPV vaccine instead of a VFC HPV vaccine. Patient denies any adverse reaction to vaccine. Attempted to contact parent twice via phone call. Parent did not answer. Unable to leave a voicemail due to mailbox not being set up at this time.
