Symptom narrative

Patient in clinic for vaccinations. Nurse administered varicella vaccine as well as a Proquad vaccine. No adverse reactions noted after 15 minute observation period. Parent able to be contacted after 2 days, states low grade fever 12/17/2025 but no other reactions noted.
