Improper storage; Vaccinated with 0.1 ml; This spontaneous report was received from a certified medical assistant on 12-Dec-2025 and refers to a 1-year-old patient of unknown gender with no illness at the item of vaccination and with no current condition experienced. The patient's medical history, and concomitant therapies were not reported. On 06-Oct-2025, the patient was vaccinated with an improper storage of Hepatitis A Vaccine, Inactivated (VAQTA) (expiration date not reported, but upon internal validation established as lot #Y018512, expiration date: 03-Jul-2026), dose: 0.5 mL for Prophylaxis (Product storage error). On the same date, the patient was vaccinated with an improper storage of Diphtheria and Tetanus Toxoids and Acellular Pertussis Vaccine Adsorbed, Inactivated Poliovirus, ... (VAXELIS), (lot #U8335AA, expiration date not reported, but upon internal validation established as 31-Oct-2027), dose: 0.1 mL. for Prophylaxis (underdose, Product storage error). Vaccinated at Clinic with public funds. Administered doses of suspect vaccines experienced the temperature excursion (TE) below 34.3 Fahrenheit degrees (F) for 3 hours 57 minutes 30 seconds on 02-OCT-2025, below 34.8 F for unknown at time of call, below34.7 F for 7 hours 45 minutes 0 seconds on 15-OCT-2025, below 35 F for unknown at time of call. The reporter called back, and TE was completed with another agent while this AE case was being worked on, TE case noted with caller stating that the two unknown dates/temps were a mistake, and that there were only two excursion dates.
