No additional AE; inadvertently administered to patient; This spontaneous report was received from a nurse and refers to a 7-month-old male patient. The patient's medical history and concurrent conditions were not reported. Concomitant medications included Influenza vaccine, diphtheria vaccine toxoid (+) hepatitis b vaccine rhbsag (yeast) (+) pertussis vaccine acellular 3-component (+) polio vaccine inact 3v (vero) (+) tetanus vaccine toxoid) (PEDIARIX), pneumococcal vaccine conj 20v (CRM197) (PREVNAR 20). On 30-Sep-2025, the patient was inadvertently vaccinated with the second dose of Haemophilus b Conjugate Vaccine (Meningococcal Protein Conjugate) (PEDVAX HIB) (lot #Y018677, expiration date: 10-Apr-2027, 1 dosage form (reported as single use vaccine), strength unknown), administered by intramuscular route in right thigh for prophylaxis after the excursion (Product storage error). No additional adverse event. Administered dose of Haemophilus b Conjugate Vaccine (Meningococcal Protein Conjugate) (PEDVAX HIB) experienced the temperature excursion below a cumulative max temp of 8.9 degree Celsius for a cumulative total of 44 minutes.
