Symptom narrative

Patient received vaccine she was not due for. Patient received her HPV which I verified with her, and she was okay with receiving it i administrated it into her left deltoid. Patient was on nurse visit for "3rd HPV" based off my training and with me still being in my 90 day grace period I thought it was okay to give to patient because it was scheduled for her "3rd" I called patient to room and verified her information, and she was okay with me giving it to her. I looked through patients' chart after I gave her the injection because I thought it was okay because it was scheduled and I than saw she hasn't received previous HPV shots according to her immunizations in her chart, so I went to immunization registry and seen she hasn't had any previously.
