Symptom narrative

Patients PCP ordered MMR and Varivax as separate vaccines. Since the patient was nervous two vaccinators were needed. one MA retrieved the Varivax while the other retreived Proquad instead of MMRII. This error was discovered after administration and the patient was already out of the facility. Staff involved immediately notified Dr. and myself (Vaccine Manager). a call was placed to the immunization program and was advised that patient should monitor for fever but other wise no concern. Dr. called and notified patients parent
