Symptom narrative

Sore left arm day after. Stiff neck and shoulders with pain, 2 days after vaccine. OTC not helping, heat or ice not helping. Dr advised an xray and ultrasound as injection area is unusually hot and swollen. Urgent Care clinic wanted to charge another copay to see me, so I refused and scheduled with my primary. Physician at urgent care said it could be infection, but never got to see him as I wasn't going to pay another copay. Paperwork said to return if adverse side effects after 3 days, but they wanted to charge me. Primary prescribed muscle relaxers and antibiotics.
