1 week ago and started on ofloxacin for right otorrhea through his PE tube. At that time, he was also given Pediarix. Two days later, he had a seizure and was seen in the ER. On Wednesday afternoon, he developed a fever of 101 degrees, which was managed with medication. His leg remained slightly swollen on Thursday, but he was otherwise well. On Friday morning, he was dropped off at daycare at 7:00 AM. By 9:30 AM, the mother received a call informing her of a high fever, prompting her to pick him up. Upon arrival, his temperature was recorded at 103.9 degrees. The mother administered ibuprofen and changed his clothes. However, he experienced a seizure lasting approximately 4 minutes during a diaper change. Post-seizure, he was unresponsive and it was unclear if he was breathing during the seizure. He was breathing post-seizure but remained unresponsive. At that time, he had a 104-degree fever, his ear drainage had resolved, but he was starting to complain of ear pain. He was found to have right acute otitis media, and his PE tube was not working at that time. He had labs done which showed CRP of 25, procalcitonin of 6.2. His white blood cell count was 29.9 with 73% neutrophils and 17% monocytes. His respiratory PCR did show human rhinovirus/enterovirus. He was diagnosed with a febrile seizure and right acute otitis media, given a dose of Rocephin. He followed up the following day. He also had a head CT after his seizure, which was normal. He was seen the following day in the ER for a second dose of Rocephin and to repeat his CBC which showed that his white count had gone down to 13.4 with 57% neutrophils and 10% monocytes. He received his third dose of Rocephin at pediatric after-hours and is here today for a recheck.
