Patient came into clinic to receive 4 month vaccinations. Patient was due to receive Vaxelis, pneumonia, and RotaTeq. The vaccinations were requested while we were, then brought from other location by individual, our physical therapist. After administrating vaccination and going to chart I realized that Hepatitis A was given instead of Vaxelis. I consulted with pharmacy to see what needed to be done regarding this error. Individual stated there should be no side effects or risks with giving this vaccination early, and that patient could come in anytime to receive the Vaxelis vaccine. The patient's dad was called and notified about this error and he was informed that there should be no side effects associated with receiving this vaccination too soon, however should be aware of these potential side effects: hives, difficulty breathing, swelling of the face/lips/tongue/throat, seizure, extreme drowsiness or fainting, crying for an hour or longer without stopping. If these were to occur, he should present to the nearest ED. Dad verbalized understanding and had no further questions or concerns. There has been no adverse effects from wrong vaccination at this time!
