Symptom narrative

Patient was in our office on 12/15/25 and got 2 vaccines. She came back on 12/18/25 to the office for a skin check regarding lesion located on the left arm. Ache left upper arm at site of Mening B vaccine (#1), worse 2 days prior to 12/18 visit. Swollen and red. No fever or other signs of illness. No meds taken for this. Patient stated that the site ached immediately after the vaccine was given. It is less red on 12/18 than it was initially. Physical exam showed no more redness. Oval 11x6cm area of mildly tender swelling of left upper arm. At the top lateral aspect of this oval is a clean, non-inflamed injection site. No redness or discharge. No fluctuance. Patient moved arm well but reported pain with movement.
