MMR II vaccine was given to patient on 12/22/25 by the CMA. After vaccine was given she went to document the vaccine and noticed she had given an expired vaccine. MMR II had expired on 12/11/25. I am the current back up vaccine coordinator for the clinic. On 12/23/25 I called the manufacturer to get instructions on what to do and what to look for. Per Merck repeat dose in 4 weeks since it is a live vaccine and monitor for side effects of fever, rash, swelling and soreness of the injection site. Provider was also notified of this, patient mother was contacted and notified of the situation. She was instructed to call/walk in our office or go to ER if any signs or symptoms appear. She has been scheduled to return on 1/20/26 for follow up.
