Symptom narrative

Administered wrong vaccine, patient was supposed to receive Gardasil 9; This non-serious case was reported by a nurse via sales rep and described the occurrence of wrong vaccine administered in a 6-month-old female patient who received HBV (Engerix B adult) (batch number 553E2, expiry date 16-JAN-2028) for prophylaxis. Co-suspect products included HPV vaccine VLP rL1 9v (yeast) (Gardasil 9) for prophylaxis. On 03-DEC-2025, the patient received Engerix B adult (left arm). The patient did not receive Gardasil 9. On 03-DEC-2025, an unknown time after receiving Engerix B adult, the patient experienced wrong vaccine administered (Verbatim: Administered wrong vaccine, patient was supposed to receive Gardasil 9). The outcome of the wrong vaccine administered was not applicable.
