The provider I am working with ordered the (Pentacel Vaccine) DTAP/IPV/Hib. I pulled out of the fridge (Vaxelis) DTAP/IPV/HEP B/Hib. I had another nurse verify vaccines and also miss verifyed. I went to the room verified vaccines with mother. Stated it was all correct and gave baby shots. I realised when charting the vaccine I had given the incorrect one.
