Symptom narrative

Initially triaged 10/29/25, reporting injection site red, tender, warm, and getting worse. Seen at UC 10/31/25 + Dx with Cellulitis. Treated with Sulfamethoxazole - Trimethoprim 800-160 mg BID. She did have reaction to treatment requiring additional UC visit 11-9-25 for drug rash.

ADHD; Hypothyroidism; HSV infection; S/P C-section (Chronic); Chronic pain of both hips; Moderate persistent asthma without complication (HCC); Vaping nicotine dependence, tobacco product; GAD (generalized anxiety disorder); Severe depression (HCC); Suicidal thoughts; Dyslipidemia; Telogen effluvium; Personal history of congenital hip dysplasia
