He received both covid and flu shot the same day regardless that he should not have signed a consent due to confusion that the facility was aware of. He started out " his head felt funny, then short of breath weak, sleeping over 20 hours difficulty walking. days before you could not get him to slow down, shaking then incontinence then difficulty swallowing his last few days then death. Until he got these 2 shots he was walking with a walker and intended to come home. We have our own medical care and he was brought immediatle to Dr's office when he was not well. I had no idea he got these shots I would never agreed to 2 shots in one day and definetly not the covid shpt. He consented to something despite facility knowing he was confused . Other family members have voiced a concern as well that there loved ones they did not have any idea they got it either. I honestly do not know what covid and flu shot he recieved but I had to select something. Facility at the [withheld city, state] would know
