Symptom narrative

Patient came for immunization catch-up with vaccine records from [withheld country]. The provider did not notice that the patient had already completed the DTaP series and ordered a Tdap to complete the series. Patient received an extra dose of Tdap. Later, the MA spoke with the mother of the patient, letting her know about the incident and that MOP did not report any adverse effect.
