the patient was given just the Shingrix diluent; the patient was given just the Shingrix diluent; This non-serious case was reported by a other health professional via sales rep and described the occurrence of inappropriate preparation of medication in a 58-year-old male patient who received Herpes zoster (Shingrix) (batch number KB2YT, expiry date 29-JAN-2026) for prophylaxis. Concurrent medical conditions included diabetes and lipids increased (high lipids). Concomitant products included simvastatin. On 04-DEC-2025, the patient received Shingrix (left deltoid). On 04-DEC-2025, an unknown time after receiving Shingrix, the patient experienced inappropriate preparation of medication (Verbatim: the patient was given just the Shingrix diluent) and inappropriate dose of vaccine administered (Verbatim: the patient was given just the Shingrix diluent). The outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were not applicable.
