it was reconstituted with sterile diluent; nurse that gave one patient only half of the Shingrix vaccine; This non-serious case was reported by a nurse via call center representative and described the occurrence of inappropriate reconstitution technique in a patient who received Herpes zoster (Shingrix) for prophylaxis. On an unknown date, the patient received Shingrix. On an unknown date, an unknown time after receiving Shingrix, the patient experienced inappropriate reconstitution technique (Verbatim: it was reconstituted with sterile diluent) and incomplete dose administered (Verbatim: nurse that gave one patient only half of the Shingrix vaccine). The outcome of the inappropriate reconstitution technique and incomplete dose administered were not applicable.
