Symptom narrative

I saw patient on 12/2 for rash concern. Parents reported onset of red, firm bump overlying Pentacel injection site 1 day after receiving vaccination. They presented due to progressively growth in size and change in appearance of lesion, as well as 2 smaller lesions with similar appearance to initial lesion. At time of visit, primary lesion described as "approx 1cm annular, raised lesion due to scabbing on L thigh with outgoing "sun rays" from ring; entirety of lesion with thin erythematous border; center of lesion is erythematous and nonscaly." Lesions were itchy and nonpainful, not responding to several days of neosporin at home. Otherwise there were no recent illnesses, no new environmental or skin exposures, no new medications, no recent travel. Rash appeared somewhat consistent with erythema multiforme. Advised watchful waiting with mupirocin and topical steroid if no improvement after a few days. At f/u visit 6 days later, rash had almost completely resolved without intervention.
